Healthier SG — What It Means for You

A simpler way to look after your long-term health

Healthier SG is a Ministry of Health national programme launched in July 2023. It matches you with one family doctor who takes the long view of your health — preventing problems, managing chronic conditions, and keeping you out of hospital where possible.

Enrolment is free. The clinic you enrol with becomes your Healthier SG family medicine clinic, and you continue to see our team there for your routine and chronic-disease care.

At Kenneth Tan Medical Clinic, we are a registered Healthier SG provider. If you are eligible and would like to enrol with us, our reception team can help you complete the enrolment in about 5 minutes during any clinic visit.

Who is eligible?

  • Singapore Citizens and Permanent Residents
  • Aged 40 and above

You do not need to be a Community Health Assist Scheme (CHAS), Pioneer Generation, or Merdeka Generation cardholder to enrol.

What you get after enrolling

1. A fully-subsidised first Health Plan consultation

This is a short, focused conversation (typically 5 to 10 minutes) where your doctor sets your health goals for the year — reviewing your history, key risk factors, and preventive priorities.

Please note this visit is specifically for setting up your Health Plan. It is not a regular consultation — no medications, investigations, or medical certificates will be issued at this appointment, and any acute or chronic concerns you have will be addressed at a separate visit. Government-approved Healthier SG screenings or vaccinations may be given at the same time if they are due.

2. Healthpoints worth $20

Credited to your Healthy 365 app after this first Health Plan consultation.

3. More affordable chronic medications

Under the Healthier SG Chronic Tier (effective February 2024), enrolled patients can receive selected chronic medications at our clinic at prices similar to polyclinic prices. For Community Health Assist Scheme (CHAS), Pioneer Generation, and Merdeka Generation cardholders, means-tested subsidies of up to 87.5% apply on the medication list — with no dollar cap at your enrolled GP clinic.

You can also use MediSave to pay for your chronic-condition consultations and medications, up to the annual withdrawal limit ($500 currently, rising to $700/$1,000 in January 2027). This is a real, month-on-month saving for most chronic-disease patients.

4. A personalised Health Plan

Shared between you and your doctor — guiding your preventive care, vaccinations, screening schedule, and chronic disease management for the year ahead.

5. Continuity

You are enrolled with one of our clinics, and our team at that clinic manages your care year after year. Our notes and records are shared across our team.

6. Nurse-led counselling

Access to nurse-led counselling for lifestyle change, diet, smoking cessation, and chronic disease self-management under the programme.

How to enrol — two ways

Option 1 — Self-enrol via the HealthHub app (~5 minutes)

  1. Download the HealthHub app (from Google Play or the Apple App Store) or visit healthhub.sg on a computer.
  2. Log in using Singpass.
  3. Tap the Healthier SG banner on the home screen.
  4. Select “Enrol to Healthier SG”.
  5. Search for “Kenneth Tan Medical Clinic” and select either our Joo Chiat or our Punggol clinic as your enrolled provider. You are enrolling at one specific clinic — if you want to change later, our staff can process the switch for you.
  6. Book your first Health Plan consultation.

Option 2 — We help you enrol at the clinic

Walk in to either our Joo Chiat or Punggol clinic and ask our reception to help. We will guide you through the HealthHub app on your phone. Bring your Singpass credentials. Enrolment typically takes 5 minutes at our counter.

What happens at your first Health Plan consultation?

A short, structured appointment — typically 5 to 10 minutes. During it, your doctor will:

  • Review your health history, family history, current medications, and any existing chronic conditions
  • Set your health goals for the year — preventive priorities, screenings due, risk factors to address
  • Co-develop a Health Plan with you for the year ahead

The first Health Plan consultation is fully subsidised under the programme — there is no out-of-pocket charge. Medications, investigations, and medical certificates are not issued at this specific appointment — these are addressed at your regular consultations. Government-approved Healthier SG screenings or vaccinations may be administered at the same visit if they are clinically due.

Why enrol with Kenneth Tan Medical Clinic?

  • Patient-centred care. We do not participate in third-party insurance panels, which lets us plan our consultation time around your clinical needs rather than insurer quotas.
  • Transparent fees. Our consultation fees are displayed at each clinic and on our website. Medication prices are shown at the dispensing counter per Ministry of Health guidelines.

What Healthier SG does NOT do

To set expectations clearly:

  • It does not cover cosmetic or elective procedures
  • It does not override your right to see any other doctor for a second opinion or one-off visit

Healthier SG is specifically about preventive care and chronic disease management in primary care — the kind of work a family doctor does best.
