2. Standard Hepatitis B Vaccination Regimen
- Vaccine Formulations: Engerix-B 10 mcg or 20 mcg (GSK)
- Standard Dosing: 3-dose series at 0, 1, 6 months (10 mcg or 20 mcg per dose depending on formulation)
- Target Groups:
- All infants (part of childhood schedule)
- Non-immunised adolescents and adults
- Healthcare workers
- Patients with chronic renal disease (special dosing)
- Response: Expected to achieve protective anti-HBs levels in >90% non-immunocompromised recipients
3. Renal Patients Double-Dose Regimen
- Indication: Patients aged 16 years or older with renal insufficiency (including haemodialysis patients)
- Dose: 40 mcg per dose (double standard dose)
- Schedule: 4-dose series at 0, 1, 2, 6 months
- Administration Options:
- Four doses of 20 mcg formulation (4 × 20 mcg)
- Four doses of 10 mcg formulation (4 × 10 mcg) with alternate administration method
- Rationale: Poor antibody response in renal patients; double-dose improves seroconversion
- NEW: Double-dose regimen now formally included in NAIS subsidy coverage (effective 1 Jan 2026)
4. Subsidy Eligibility
- CHAS Clinics: Can now claim subsidies for renal patients on double-dose regimen using approved formulations
- NAIS Inclusion: Double-dose regimen newly included in National Adult Immunisation Schedule
- MediSave: May be applicable; verify current arrangements
- Claims Process: CHAS GPs to submit claims using appropriate codes for double-dose regimen
- Prior Appeal System: Previous FAQs on appeal-based subsidy claims superseded by formal NAIS inclusion
5. Action for Your Practice
- Identify renal patients (age 16+) on your panel requiring hepatitis B vaccination
- Screen for prior hepatitis B serology; vaccinate if non-immune (anti-HBs negative)
- For renal patients: use double-dose regimen (40 mcg, 4-dose schedule)
- Document renal insufficiency status clearly in patient records for subsidy claims
- Train staff on correct dosing/scheduling to avoid claiming standard dose
- For non-renal patients: continue standard 3-dose regimen
